Through this website, no data is collected.  Once a treatment relationship has been established, patient healthcare information will be securely stored in the electronic medical record.  This information is customary, and includes demographics, billing/insurance/pharmacy preferences, and office notes. Common practice forms, including consent for treatment, privacy acknowledgement and release of information forms will be stored within the same electronic medical record. Inquiries for copies of medical records can be arranged by contacting your provider.
